 Registration Agreement

My child has permission to participate in various dance classes at Maxine Patterson School of Dance (MPSOD) in Chatham County.

 I hereby waive and release the Maxine Patterson School of Dance (MPSOD) and all instructors at the studio of any claims, actions, causes of action,  illness, personal injury, level of risk of COVID-19, loss or damages, costs, liabilities, expenses or judgments, including attorney's fees and court costs, arising out of my child's participation in this program

I understand that tuition must be paid by 10th of each month (Savannah) or by cut off dates (Pooler Recreation Center) or a late fee of $25.00 will be added. There is a $25 fee for any returned check. No further checks will be accepted if this happens. We do not accept post dated checks.

I understand  that I will pay  in full all account balances including late fees, costume fees, ticket fees, etc...

I understand that there will be no refunds or credits made for absences or withdrawal from class. Uncollected balances including late fees, check fees, and all other fees will be turned over to a collection agency and/or attorney for collection.

I understand that if I am more than 15 minutes late picking my child up, there will be a $5.00 charge for each additional 15 minutes or any part therof. This will be added to my dance account and must be paid

I understand that if my child wishes to discontinue any classes that  I will notify at any time by providing 7 days written notice or by  phone to the owner, Maxine Patterson of this decision.
